Why don't republicans want to bring actual reform to health care?

People with preexisting conditions that need expensive medical care and the poor that go to emergency rooms for basic medical care are the most expensive problems with the US health care system. Republicans are ignoring this outright and are repeating the "tort reform" mantra. Tort reform won't fix health care's largest problems. So why don't republicans want to do more?
